Personal service setting special event vendor notification form

Personal service providers planning to offer services at special events must receive approval from Southeast Public Health. This includes hairdressing, tattooing, manicures, pedicures, aesthetics, and other personal services. Complete and submit the Personal Service Settings Special Event Application Form at least 14 days prior to the event.

All vendors providing a personal service at a special event must comply with Ontario Regulation 136/18: Personal Service Settings under the Health Protection and Promotion Act. Vendors are encouraged to review the personal service setting special event guidelines prior to submitting the application.

Completed form to be submitted to Southeast Public Health fourteen (14) business days before the event.
