STBBI medications order form

This form is for STBBI medications orders. These medications are to be used for the treatment of laboratory-confirmed and clinically diagnosed cases, and contacts of confirmed cases of chlamydia, gonorrhea, and syphilis. Authorization for larger orders will be granted on an individual basis. If you require a STI drug not listed on this form, please contact us at 613-549-1232, extension 2000 (Inventory)

Order deadlines and pick-up times

Orders being picked up at the Belleville, Kingston, or Brockville offices are ready three business days (72 hours) after the order is submitted. Please note the following office hours:

  • Belleville: Monday to Friday 8:30 a.m. to 4 p.m.
  • Kingston: Monday to Friday 8:30 a.m. to 4:30 p.m.
  • Brockville: Monday to Friday 8:30 a.m. to noon and 1 to 4 p.m.

Orders being picked up at any other office must be submitted by noon on Wednesdays for pick up the following week. Please note the following pick-up days and times:

  • Trenton: Monday afternoon to Wednesday 8:30 a.m. to noon and 1 to 4 p.m.
  • Picton: Monday afternoon to Friday 8:30 a.m. to 4 p.m.
  • Napanee: Monday afternoon to Friday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Gananoque: Tuesday 8 a.m. to noon and 1 to 4 p.m.
  • Smiths Falls: Monday afternoon to Friday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Almonte: Tuesday and Thursday 9 a.m. to noon and 1 to 4 p.m.
  • Kemptville: Monday afternoon to Thursday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Bancroft: Tuesday and Thursday 8:30 a.m. to 4 p.m.

Orders not picked up by end-of-day on Fridays may be returned.
