Report a disease of public health significance
Under the Health Protection and Promotion Act, all health-care providers, directors of laboratories, and administrators of hospitals and schools are legally obligated to report a suspect or confirmed case of a disease of public health significance (reportable communicable disease) to their local Medical Officer of Health.
How to report
Access the Diseases of Public Health Significance Reporting Form and select the infection you wish to report. This form will provide instructions for reporting to Southeast Public Health.
If indicated, report immediately by telephone.
Diseases that do not require immediate notification must be reported by the next business day using the online reporting form, OCEAN, or fax.
