Seasonal respiratory vaccine order form

This order form is for influenza, RSV and COVID-19 vaccine orders. For quality assurance purposes, you must submit your fridge temperature log/s and vaccine doses in stock to complete your order.

Here are a few reminders to help your order go through smoothly:

  • Orders are processed based on product availability and will be filled as soon as supplies are ready.
  • To help avoid delays, please include contact information for the best person we can reach out to if any follow-up is needed.

Order deadlines and pick-up times

Orders being picked up at the Belleville, Kingston, or Brockville offices are ready three business days (72 hours) after the order is submitted. Please note the following office hours:

  • Belleville: Monday to Friday 8:30 a.m. to 4 p.m.
  • Kingston: Monday to Friday 8:30 a.m. to 4:30 p.m.
  • Brockville: Monday to Friday 8:30 a.m. to noon and 1 to 4 p.m.

Orders being picked up at any other office must be submitted by noon on Wednesdays for pick up the following week. Please note the following pick-up days and times:

  • Trenton: Monday afternoon to Wednesday 8:30 a.m. to noon and 1 to 4 p.m.
  • Picton: Monday afternoon to Friday 8:30 a.m. to 4 p.m.
  • Napanee: Monday afternoon to Friday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Gananoque: Tuesday 8 a.m. to noon and 1 to 4 p.m.
  • Smiths Falls: Monday afternoon to Friday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Almonte: Tuesday and Thursday 9 a.m. to noon and 1 to 4 p.m.
  • Kemptville: Monday afternoon to Thursday 8:30 a.m. to noon and 1 to 4:30 p.m.
  • Bancroft: Tuesday and Thursday 8:30 a.m. to 4 p.m.

Orders not picked up by end-of-day on Fridays may be returned.
